Heel Pain & Plantar Fasciitis

Heel pain is one of the most common foot complaints we see, and one of the most treatable. Plantar fasciitis โ€” inflammation of the thick band of tissue running along the bottom of your foot โ€” is the leading cause of heel pain in adults. It typically presents as sharp pain in the heel, especially with your first steps in the morning or after sitting for long periods.

At Dr. V, DPM, we evaluate the root cause of your heel pain and develop a targeted treatment plan that may include custom orthotics, physical therapy guidance, anti-inflammatory treatments, cortisone injections, or in resistant cases, minimally invasive procedures. Most patients experience significant relief without surgery.

Don't let heel pain limit your daily life. With the right treatment, most patients are back to full activity within weeks.

Diabetic Foot Care

For patients living with diabetes, foot health is a critical part of overall wellbeing. Diabetes can reduce circulation and nerve sensation in the feet, making it harder to notice injuries โ€” and making those injuries harder to heal. Without proper monitoring and care, minor problems can escalate to serious complications.

Dr. V provides comprehensive diabetic foot care including regular exams, circulation and nerve assessments, wound evaluation, and proactive treatment to prevent ulcers and infections. For patients with active wounds or advanced complications, Dr. Vinogradov's expertise in diabetic limb salvage means you have a specialist committed to preserving your limb and your quality of life.

If you have diabetes, annual foot exams are not optional โ€” they're essential. We make it easy to stay on top of your foot health at any of our three East Tennessee offices.

Ingrown Toenails

Ingrown toenails occur when the edge of a nail grows into the surrounding skin, causing pain, redness, and swelling. If left untreated, they can become infected. While mild cases may respond to at-home care, recurring or infected ingrown toenails need professional treatment.

Dr. V performs quick, comfortable in-office procedures to remove the ingrown portion of the nail and, when necessary, permanently prevent it from growing back. The procedure is performed under local anesthesia and most patients walk out of the office with minimal discomfort. Recovery is fast and the results are lasting.

You don't need to live with this pain. Same-day appointments are often available for urgent ingrown toenail cases.

Bunions & Foot Deformities

A bunion is a bony bump that forms at the base of the big toe, causing the toe to angle toward the others. Over time, bunions can cause significant pain, difficulty finding comfortable shoes, and problems with balance and gait. Hammertoes, mallet toes, and other deformities can develop alongside bunions or independently.

Treatment depends on severity. Dr. V begins with conservative measures โ€” wider footwear, orthotics, padding, and anti-inflammatory care โ€” before considering surgery. When surgery is needed, Dr. Vinogradov's expertise includes minimally invasive techniques that allow most patients to walk immediately after surgery โ€” and some patients are back in a tennis shoe in as little as 3 weeks.

Early treatment tends to produce the best outcomes. If you're noticing a growing bump on your foot or your toes are beginning to crowd, schedule an evaluation sooner rather than later.

Wound Care

Chronic wounds, diabetic ulcers, and non-healing injuries require specialized attention. Standard wound care isn't always enough for wounds that persist or worsen. Dr. Vinogradov has advanced training in wound care and diabetic limb salvage โ€” the goal of preserving limbs that might otherwise face amputation.

Treatment may include debridement, advanced wound dressings, offloading devices, infection management, and coordination with vascular specialists when necessary. As a surgical specialist himself, Dr. Vinogradov is equipped to manage complex wound cases that require operative intervention โ€” keeping your care under one roof whenever possible. We take every wound seriously because we understand what's at stake.

If you or a loved one has a wound that isn't healing, don't wait. Early intervention dramatically improves outcomes.

Sports Injuries

Foot and ankle injuries are among the most common sports injuries, whether you're a competitive athlete or a weekend warrior. Ankle sprains, stress fractures, Achilles tendon problems, plantar fasciitis flare-ups, and turf toe are just a few of the conditions Dr. V regularly treats in active patients.

Our approach is to get you back to activity as safely and quickly as possible. That means accurate diagnosis โ€” including imaging when needed โ€” followed by a targeted treatment plan. For some patients that's conservative care; for others it may include in-office procedures or surgery.

We treat athletes of all ages and skill levels. From youth sports injuries to adult recreational activity, Dr. V understands how much getting back on your feet matters.

Nail Fungus

Fungal toenail infections (onychomycosis) cause nails to become thickened, discolored, brittle, and sometimes painful. They're more common than many people realize and can be persistent without proper treatment. Over-the-counter products rarely cure established nail fungus.

Dr. V offers effective treatment options for nail fungus, including prescription topical treatments, oral antifungals, and when appropriate, nail removal. We'll evaluate the severity of your infection and recommend the most effective approach for your specific situation.

Clear, healthy nails are achievable with the right treatment plan. Don't be embarrassed โ€” this is a very common condition and we've seen it all.

Foot & Ankle Fractures

Fractures of the foot and ankle range from minor stress fractures to complex multi-bone breaks requiring surgical fixation. Whether you rolled your ankle, dropped something on your foot, or developed a stress fracture from repetitive activity, prompt and accurate diagnosis is critical to proper healing.

Dr. V performs comprehensive fracture evaluation including X-rays and advanced imaging when needed. Treatment options range from casting, bracing, and offloading for stable fractures to open reduction and internal fixation (ORIF) for displaced or unstable breaks. The goal is always to restore alignment, stability, and full function.

Don't walk off a foot injury hoping it will heal on its own. Improperly healed fractures can cause long-term pain, arthritis, and instability. Get it evaluated right the first time.

Minimally Invasive Surgery (MIS)

Minimally invasive foot and ankle surgery uses specialized instruments and small incisions to correct conditions that once required large open surgeries. Patients benefit from less tissue disruption, reduced pain, faster recovery, smaller scars, and earlier return to activity.

Dr. Vinogradov is trained in minimally invasive surgical techniques for a range of conditions including bunions, hammertoes, heel spurs, and certain fractures. Not every condition is a candidate for MIS, but for those that are, the results can be dramatically better than traditional open approaches.

During your evaluation, Dr. V will assess whether a minimally invasive approach is appropriate for your specific anatomy and condition, and walk you through what the procedure and recovery would look like.

Plantar Warts

Plantar warts are caused by the human papillomavirus (HPV) and grow on the bottom of the foot, often in pressure areas. They can be painful, persistent, and frustratingly resistant to over-the-counter treatments. Some warts cluster into larger lesions called mosaic warts, which can significantly impact daily comfort.

Dr. V offers effective in-office treatments for plantar warts including topical acids, cryotherapy, and surgical excision for warts that don't respond to conservative measures. We'll choose the approach based on the wart's size, location, and your treatment history.

Don't let a wart that can be treated in a single visit linger for months. Come in and we'll take care of it.

Diabetic Shoes

Diabetic shoes โ€” also called therapeutic shoes โ€” are specially designed footwear prescribed for patients with diabetes to help prevent serious foot complications. Standard shoes can create pressure points that lead to blisters, ulcers, and infections in patients with reduced sensation or poor circulation. Diabetic shoes reduce this risk significantly.

Dr. V prescribes Medicare-covered diabetic shoes and custom inserts for qualifying patients as part of a comprehensive diabetic foot care plan. Most patients with diabetes who meet the criteria can receive one pair of therapeutic shoes and up to three pairs of custom inserts per calendar year at little or no cost through Medicare.

Orthotics & Bracing

Custom orthotics are prescription medical devices that fit inside your shoe to correct how your foot functions. Unlike store-bought insoles, custom orthotics are designed from a mold or scan of your specific foot and are built to address your individual biomechanical issues โ€” whether that's flat feet, overpronation, leg length discrepancy, or offloading a painful area.

Ankle-foot orthoses (AFOs) and other braces can provide stability, reduce pain, and prevent further injury for patients recovering from surgery, managing chronic conditions, or dealing with ankle instability. Dr. V works with each patient to determine what level of support is needed and which device will best fit their lifestyle and footwear.

Orthotics can be transformative for patients with chronic foot pain who haven't found relief with other treatments. If you're on your feet all day and struggling, this may be exactly what you need.

โœ… Custom orthotics are FSA and HSA eligible โ€” you can use your Flexible Spending Account or Health Savings Account to cover the cost.

Ankle Pain & Sprains

Ankle sprains are the most common musculoskeletal injury, but they're not always "just a sprain." Improperly treated ankle sprains can lead to chronic instability, recurring injuries, and long-term pain. If your ankle doesn't feel right after an injury โ€” or you've been dealing with chronic ankle pain โ€” it's worth getting a professional evaluation.

Dr. V performs comprehensive ankle assessments including physical examination and imaging when needed. Treatment plans are individualized and may include bracing, physical therapy guidance, injections, or surgical stabilization for chronic instability cases.

We also treat a full range of non-injury ankle conditions including arthritis, tendinitis, and nerve-related ankle pain. Whatever is slowing you down, we can help.
